what if your loved ones wave their hands and begging you to save them. Would you turn away from them and save the 1 billion strangers? Letting your loved ones die and saving strangers instead is very cruel. I would never do this to my loved ones. Loved ones deserve better than that.
I think it's better to assume the X number of people have no clue of what is going on while your loved ones know the situation. What ever you do, no one know excepts your loved ones.
For me, The X doesn't exist because no matter how large it is, my loved ones come first. Imagine if you are in trouble, who would help you out? Your loved ones or the billion strangers?

Save the X number of people, and the person's emotional health will be damaged.

Why would I suffer for doing the right thing??? What do I owe these strangers? Nothing. I didn't do anything to hurt them, why should I feel guilty of anything?

My solution is save my loved ones first then try to save those strangers later if I can.
i don't give a rat's @ss about strange people. plus, the death of many millions of them will be good for the earth and humanity in the long term as there'll be more living space and less natural resource consumption.

I would feel bad either way I choose, but it would hurt worse without my loved one. So, I would save my loved one over X. Sorry X.

Why would you have to kill yourself? I think you are confused. We are not talking about you kill those innocent literally. We are talking about choosing your loved ones and yourself over the strangers. I wouldn't lose any sleep for saving myself and loved ones over those strangers. No law in the world can go after me for doing it.
