Asada Ryutaro, Ijyuuin Noboru and Fujiyoshi Keisuke defy Professor Noguchi over the treatment of the patient, Nishizawa Takafumi, and are relegated to Hokuyou Hospital. Of the former team, only Arase Monji and Satohara Miki remain at Meishin Medical University Hospital. However, Asada proclaims to Zenda Hideki, the director of Hokuyou Hospital, that he will form a new team.
Zenda, who has never been pleased with the objectives of Hokuyou Hospital owner, Kataoka Kazumi, and Professor Noguchi, declares before the two of them that Asada will change Hokuyou. However, they have arranged for Kitou Shoko to take Asada's place in their vision of a medical city as Meishin's newly appointed director; moreover, the plan to quietly destroy Hokuyou is already underway.
At that moment, Ijyuuin is dumbfounded by the listlessness of the doctors working at Hokuyou. The anesthetist Kodaka Nanami and surgeon Toyama Seiji, both of whom had been transferred from Meishin together with Ijyuuin, Asada and Fujiyoshi, are no different. Matsudaira Kotaro, who has been with Hokuyou for since the past, spends his days drinking in the department.
While Ijyuuin is disconcerted by what he sees, a patient is brought in. It is Nishizawa. Asada immediately brings Nishizawa to the ICU and performs an emergency surgery. Nishizawa's pre-surgical evaluation had been done at Meishin. Ijyuuin recruits medical staff for the surgery but Kodaka won't budge. Toyama joyfully comes to the operating theatre but says that he wants to be the lead surgeon. Asada makes Toyama his first assistant and the surgery begins.
On the other hand, Fujiyoshi obtains a copy of Nishizawa's surgery records from Meishin through Miki. He sees the reason for Professor Noguchi's opposition to further surgery for Nishizawa from the records. Watching from the observation room, Fujiyoshi knows this surgery's difficulty.
Asada, who has opened up Nishizawa's chest, proceeds with a synechiotomy on the part he believes to be a tumour. When he realises that the young anesthetist cannot keep up with the surgery, he orders Ijyuuin to call Kodaka. In the meantime, Asada continues with the surgery but as he probes, a gauze emerges from the part he had thought to be a tumour. Nishizawa's post-surgical disorder is due to gauzeoma (granuloma caused by a gauze meant to stop bleeding during surgery being left behind in the body). Asada changes the operation to remove the gauzeoma. Even Fujiyoshi is amazed by his precision in separating the gauze. When it comes to the ventricular apex, Toyama offers to take over from Asada. Asada lets Toyama take his turn and he is about to separate it when that happens. Bucking ... Nishizawa's body is jerking wildly because the drug hasn't taken complete effect as a result of the anesthetist's inexperience. Toyama damages the ventricular apex and there is extensive bleeding.
Ijyuuin brings a reluctant Kodaka to the operating theatre. Kodaka is unmoved by the patient's sudden change in condition. Ignoring her, Asada calmly proceeds with the repair. Witnessing his work, Kodaka quietly starts issuing instructions to the young anesthetist. Nishizawa's operation comes to an end with Asada's prompt actions. Kodaka's actions didn't escape him.
Kitou, who has come with Kataoka to Hokuyou to watch the surgery, talks to Asada. She asks if he really intends to give up the vision of building Meishin's medical city to work at Hokuyou. Asada says there are patients that need him here. Kitou, who endorses Professor Noguchi's vision, tells Asada that it is a waste of his talent to be with Hokuyou. The aim of healthcare should be to save 10,000 people 10 years later rather than the patient right before one's eyes. Rot away with this hospital, she says as her parting words. Asada doesn't reply.
Asada, Ijyuuin and Fujiyoshi have a conference after the surgery. He shows them the surgery records Miki had brought from Meishin. It is evident that the records had been altered. Although the surgeon was another person, they deduce that the actual surgeon was Professor Noguchi due to the large number of nurses and assistants. That was why Professor Noguchi had persistently disregarded Nishizawa's appeals after the surgery. An irate Ijyuuin and Fujiyoshi decide to tell Nishizawa the truth. The next step will be a medical inquest of great importance.
The next day, Ijyuuin, Fujiyoshi and Asada head to Nishizawa's hospital room with a sense of determination. However, Kataoka is there. She says that she has come with a message for Nishizawa from Professor Noguchi, and carries an envelope of money. Ijyuuin and Fujiyoshi are stunned at Kataoka's attempt to take a preemptive measure to silence the patient. But because Nishizawa remains silent, the money is eventually accepted.
That night, Asada comes to examine Nishizawa. The old man thanks him for the surgery and tells him that his wife, who had cared for him during his surgery eight years earlier, hasn't been able to go out of the house lately. He wants to recover for his wife's sake.
Some days later, Nishizawa is discharged from the hospital in good condition. While Kataoka is at Meishin triumphantly telling Professor Noguchi that she has settled the matter with money, Asada, Fujiyoshi, Ijyuuin and Zenda are sending Nishizawa off as he walks away with his grandson, Eita. When they return to the hospital, they pass by Kataoka. Fujiyoshi, who calls out to Kataoka to stop, returns the money Nishizawa had passed to him. Kataoka's expression is full of disbelief.
On the other hand, at Meishin, Kitou orders Arase to follow her instructions and to refrain from having any contact with Asada from now on. Arase tells her that he would like to go to Hokuyou to watch them conduct a surgery because they're an affiliated hospital. Kitou talks about Nishizawa's surgery and dismisses the surgical team as useless. When she mentions that a female anesthetist was there, Arase bursts out laughing. What is the significance of his laughter?
